Form W-4 Required for new employee setup and for employee changes to federal income tax withholding.
Form I-9 Required for new employee. Be sure to obtain copies of supporting documentation, which are outlined on page 5. In addition, do not file with personnel records, but rather in separate location which is easily accessible.
Form W-11 Required for new employee who makes you eligible for the HIRE Act tax break.
Form W-5 Required for any employee who requests the Advance EIC payment.
Direct Deposit Authorization Required from any employee who wants direct deposit or needs to make changes on existing direct deposit.
